Must be able to work flexible work-schedule including some weekends and evenings.

A demonstrated ability to establish an effective working relationship with NFP families and other agencies; plan and implement effective program procedures to include participation in community coalition in support of NFP, participates in promoting public awareness of NFP; and recognize and assure a high degree of quality and efficiency in program service delivery.

Must complete all required NFP initial education.


Job Description:

NURSE HOME VISITOR:  The Nurse-Family Partnership (NFP) nurse home visitor is responsible for providing comprehensive nursing services to women and their families who meet eligibility for the Nurse-Family Partnership Program (NFP Program).  The Nurse Home Visitor is

 responsible for maintaining the highest level of standards in clinical nursing practice and adherence to the NFP model, NFP Visit-to-Visit Guidelines and standards of NFP and YDI policies and procedures.

JOB CHARACTERISTICS/PHYSICAL DEMANDS:

This position is exempt from overtime provisions of the Fair Labor Standards Act.  This is a professional position which requires excellent organizational and communication skills, as well as judgment, tact and diplomacy in contacts with program clients, employees, and community representatives 

Strict adherence is required to all regulations concerning confidentiality.

Background investigation to include fingerprinting will be required.  

Physical, including recent COVID-19 negative test, is required upon hire, and may be required intermittently.       

Frequent travel is required by employee.  Valid NM driver’s license and proof of satisfactory car insurance is required.

EXAMPLES OF DUTIES & RESPONSIBILITIES (Not Inclusive):

  • Provides home visits to women and their families eligible for NFP program in accordance with NFP model and NFP Visit-to-Visit Guidelines (eGuidelines).
  • Adheres to nursing process and NFP model of home visitation.
  • Carries a case load of up to 25 clients.
  • Follows nursing process in implementing NFP into nursing practice.
  • Develops therapeutic relationships, utilizes concepts of reflection and motivational interviewing with women and their families in a home visiting environment.
  • Assesses physical, emotional, social and environmental strengths and risks of women and their families as they relate to the NFP domains.
  • Assists women and their families in establishing goals and outcomes.
  • Provides education, support and referral resources in assisting women and their families attaining their targeted goals.
  • Consults and collaborates with others professionals involved in providing services to women and their families.
  • Formulates nursing diagnosis based on nursing assessment and client goals.
  • Evaluates client progress toward stated goals and NFP outcomes.
  • Actively engages in professional development to meet all NFP nurse home visitor competency requirements.
  • Meet with NFP nurse Supervisor weekly for clinical supervision.
  • Attend information to support staff for timely and accurate data input to the designated data system.
  • Participate in review and analysis of ETO reports for achievements and areas for improvement.
  • Participate in quality improvement efforts.
  • Represent the NFP vision, mission, and model un actions and verbally to both internal and external customers and colleagues.
  • Performs other related duties as assigned.
