RN Care Manager I/II

Join Our Care Management Team

Sunshine Community Health Center (SCHC) is seeking a Registered Nurse to join our Care Management team as an RN Care Manager I or RN Care Manager II.

This position combines direct nursing care with care coordination, transitions-of-care follow-up, patient panel management, and interdisciplinary teamwork to help patients receive timely, coordinated, and well-documented care.

Both levels perform the same core care management functions. The RN Care Manager I is a developing care manager who works with guidance from the RN Care Manager III. The RN Care Manager II demonstrates greater proficiency and independence and may train or mentor care management staff, facilitate team workflows, and provide lead-support to the RN Care Manager III. Level placement is based on demonstrated care management proficiency and experience.

What You'll Do

• Conduct in-person and telehealth care management visits, including enrollment assessments, chronic disease management visits, and follow-up visits within RN scope of practice.
• Perform clinical assessments, medication reconciliation, patient education, and individualized care planning in collaboration with primary care providers.
• Manage an assigned care management patient panel across payer types and risk tiers.
• Review and clinically triage admission, discharge, and emergency department alerts and coordinate timely transitions-of-care follow-up.
• Complete required post-discharge interactive contacts, symptom monitoring, medication review, and follow-up coordination within required timelines.
• Monitor scheduling needs, active care management episodes, documentation requirements, and care plan updates.
• Identify quality gaps and support care-gap closure within RN scope of practice.
• Participate in daily Care Management team huddles and help coordinate communication across Care Management, Patient Services, Billing, and other teams.
• Support quality, compliance, and reporting activities through accurate documentation and data collection.
• Coordinate patient resources, referrals, transportation, interpreter services, and other care-related needs in collaboration with the interdisciplinary team.

Required Qualifications

• Active, unencumbered Registered Nurse (RN) license in the State of Alaska, or an active multistate compact RN license recognized in Alaska.
• Current BLS (Basic Life Support) certification.
• Demonstrated ability, appropriate to assigned level, to perform patient assessments, develop care plans, and exercise clinical judgment within RN scope of practice.
• RN Care Manager II: Minimum of 2-3 years of RN clinical experience. Experience in ambulatory/primary care, care management, or a related setting is strongly preferred.
• RN Care Manager I: Appropriate for an RN who meets licensure and competency requirements and is developing care management proficiency.

This role requires the ability to manage multiple priorities in a busy clinical environment, maintain accurate documentation, protect patient confidentiality, and collaborate effectively with both clinical and non-clinical team members.

Experience with transitions of care, care coordination, ambulatory or primary care, chronic disease management, or related nursing workflows is especially valuable.

Employment Health Requirements

To support the health of the community, patients, and staff, SCHC requires proof of completed vaccine series or serologic test results for MMR (Measles, Mumps, and Rubella), Varicella (Chicken Pox), Tdap, and Hepatitis B, as well as TB screening, prior to employment.
