Patient Advocate, Transportation

Sunshine Community Health Center is seeking a Patient Advocate, Transportation to help patients access the care they need by coordinating local and out-of-area transportation and travel resources.

Key Responsibilities

• Coordinate patient transportation, flights, lodging, food, and other travel resources as appropriate.
• Build and maintain ride schedules and travel documentation in the electronic medical record.
• Serve as a liaison between patients, care teams, transportation providers, and outside organizations.
• Complete travel vouchers and maintain accurate records and tracking.
• Help patients understand SCHC resources and processes that support their care.
• Support care management and quality improvement activities.

Minimum Qualifications

• High school diploma or equivalent.
• Two years of appropriate office experience, including at least six months performing similar duties.
• Strong communication, organization, problem-solving, and follow-through skills.
• Ability to manage multiple priorities and work effectively with patients and multidisciplinary teams.

GENERAL

To ensure the health of our community, patients, and staff, SCHC requires proof of completed vaccine series or serologic test results for MMR (Measles, Mumps, and Rubella), Varicella (Chicken Pox), Tdap, and Hepatitis B, as well as TB screening, prior to employment.
