SUMMARY

The Referrals Coordinator manages and tracks patient referrals to internal and external providers, ensuring timely and accurate processing in collaboration with care teams. This role requires attention to detail, strong organizational skills, and the ability to work effectively with patients, providers, and colleagues. The position also participates in cross-training across Patient Services functions, including Reception and Medical Records.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Referral Management

  • Receive and process referral requests via the patient’s electronic health record (EHR).
  • Manage patient referrals, excluding veteran-specific referrals, and coordinate with relevant staff for department-specific referrals.
  • Maintain ongoing tracking and documentation of all referrals.
  • Ensure accurate data entry and recordkeeping in the EHR referral platform.
  • Manage records for deceased patients and maintain shred documentation for hard charts.

Team Collaboration & Communication

  • Work collaboratively with all SCHC care teams and within the Patient Services team.
  • Maintain positive working relationships with staff across all departments.
  • Attend staff meetings and contribute to team workflow discussions.
  • Keep the supervisor informed of any issues related to patients, staff, or referral processing.

Cross-Training & Administrative Support

  • Perform cross-training duties in Reception and Medical Records.
  • Maintain a desk procedure manual and ensure departmental processes are documented.
  • Perform other duties as assigned.

EDUCATION AND/OR EXPERIENCE

Graduation from high school or the equivalent. Two years of appropriate office work experience, including at least six months performing duties like most of those specified above.

GENERAL

To ensure the health of our community, patients, and staff, SCHC requires proof of completed vaccine series or serologic test results for MMR (Measles, Mumps, and Rubella), Varicella (Chicken Pox), Pertussis, and Hepatitis B, as well as TB screening prior to employment.
