SUMMARY:  

All Staff positions at SCHC will strive to provide patient centered, team-based, and trauma sensitive health care. Patient Advocate II team members will perform the focused, essential duties and responsibilities outlined in their job description. The primary role of SCHC’s Patient Advocates is to connect patients to appropriate resources based on their barriers to care and health goals. The outreach portion of this position is involved in community health care events to educate our service area’s catchment about Sunshine Clinic’s medical, dental, behavioral health, and care management services for each service location. This position also acts as the primary contact person for any domestic violence (DV) victim issues and collaborates with SCHC Behavioral Health to support the most appropriate resources for a possible DV crisis. 

 ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. This position is expected to be the primary source for financial eligibility assistance.
  2. Meet with all new patients to assess their health literacy, their Social Determinants of Health (SDOH) needs and explain the services Sunshine provides to include the Sliding Fee Scale, 3 Bears (340B) medication program and/or Needy Meds Patient Assistance Program and SCHC Chronic Care Management Program.
  3. Assist patients with Medicaid, Medicare, Social Security/Disability, Heating Assistance, Senior Benefits, hospital bill forgiveness, and grants and follow up on any denials.
  4. Connect patients with resources such as the local Food Banks, Meals on Wheels, and Families in Crisis.
  5. Distribution of information and coupons to eligible seniors for Farmers Markets (summer).
  6. Coordinate Medicaid travel authorizations with transportation companies and obtain approval through Conduent Travel for relevant appointments.
  7. Provide short term, non-clinical coordination to patients as it relates to other social services, i.e., AK Legal, Medicaid Waivers, Miller’s Trusts, Valley Residential and Weatherization.
  8. Collaborate to identify needs and priorities of patients and communities and to participate in interventions that help to fulfill the identified needs as they relate to outreach.
  9. Serve as a liaison between the patients and communities we work with, as well as the available health care and social services in the area.
  10. Assist patients in completing medical intake, consent and financial forms as needed with appropriate health literacy approach.
  11. Per patient situation, coordinate appointments to help facilitate completion of insurance forms, assist in state ID replacement, PFD paperwork, PCA referrals and DME.
  12. Check for incoming correspondence daily to include teams, email, alerts, flags and the patient portal.
  13. Participate in Outreach for open enrollment for Social Security, Marketplace Insurance (CAC), and Homeless Connect.
  14. Certified Application Counselor (CAC) – completion of yearly certification training and testing to remain a CAC to help patients with Marketplace Insurance.
  15. Collaborate in organizing and participating in community health care events.
  16. Perform filing for necessary documents, including incoming mail correspondence.
  17. Participate in monthly staff meetings, action team meetings, and other SCHC activities.
  18. Participate in relevant training/educational opportunities for personal/professional development.
  19. Maintain confidentiality, both regarding patient and employee information.
  20. Maintain a desk procedure manual.
  21. Complete additional responsibilities related to patient advocacy and outreach as requested.

EDUCATION and/or EXPERIENCE:  

High school diploma or GED required. A minimum of 2 years of community work experience. Current driver’s license and proof of liability insurance required.
