Physical Therapist

Description:
Provide rehabilitation services for those clients in need of physical therapy under the direction of a physician.

Primary Functions:

Primary Functions:

       
  1. Develop treatment plan including goal setting (in consultation with physician to achieve goals).
  2. Provide treatment in accordance with plan.
  3. Utilize equipment required to increase the client's function and independence.
  4. Re-evaluate patient as appropriate.
  5. Revise plan as necessary.
  6. Prepare clinical and progress notes in a timely manner.
  7. Provide for discharge planning and communicate plans to case manager RN.
  8. Consults with clients and families and VNA staff as appropriate.
  9. Holds weekly conferences with case manager RN for each patient and documents same.
  10. Participates in Quality Improvement activities.
  11. Makes appropriate interorganization and intraorganization referrals.
  12. Assumes responsibility for the PTA as appropriate and supervises PTA and HHA as appropriate.
  13. Participates in inservice programs.
  14. Familiarizes and complies with all VNA policies and procedures.
  15. Meets standards of behavior expectations.
  16. Follows established guidelines for use and/or disclosure of protected health information. Employees should report any breaches of the Health Insurance Portability and Accountability Act (HIPAA) rules to the Privacy Officer (Director of Quality, Education & Risk Management) immediately. Failure to comply with HIPAA policies and procedures will result in disciplinary action, up to and including termination of employment.
  17. Other duties as assigned.

Qualifications

Qualifications:

  1. Licensed in the State of Illinois and meets the Federal Conditions of Participation for Home Health Agencies (USC 42 Section 1395 x Health Insurance for the Aged Act).
  2. Three years experience.
  3. Maintains current CPR certification.

1.Ability to perform tasks involving physical activity which includes heavy lifting and extensive bending and standing.
2.Ability to communicate effectively, verbally and in writing.
3.Tactile, visual, auditory, and olfactory senses in tact.
4.Ability to climb stairs and ambulate in and out of patient's home in all weather.
5.Health policies and requirements are met (See Personnel Policies).
6.Ability to drive a car.
Manual dexterity sufficient to perform PT procedures
