Social Worker, LCSW - East Aurora School Based Health Center

Description:

Assesses the social, emotional and psychological development of students at the SBHC, provides intervention or referral as needed.

 


Primary Functions:

  1. Assesses students for appropriate social and emotional growth. Assess mental status including mood disorders and mental illness.
  2. Develops a plan of treatment in consultation with the nurse practitioner or medical director when treatment is indicated.
  3. Initiates interventions identified in the student’s plan of care, evaluates effectiveness and outcomes of care and revises as needed.
  4. Provides supportive psychotherapy for students as needed. Provides group therapy as appropriate.
  5. Participates in referring students to other mental health care services as needed and completes follow up on referrals per applicable policies and procedures.
  6. Conducts staffing involving students, parents, health center staff, school faculty and outside referral sources as needed. Obtains appropriate consents according to policies and procedures.
  7. Protects the rights and confidentiality of the students seen at the School Based Health Center.
  8. Maintains and monitors quality of student records and ensures inclusion of all appropriate findings, recommendations, follow-up care and referral.
  9. Participates in a team approach to student care and provides for effective communication to other professional staff to ensure continuity of care.
  10. Participates in quality assurance activities of the organization.
  11. Participates in continuing education programs, voluntary and mandatory to ensure quality care.
  12. Assumes additional responsibilities and performs other duties as appropriate and approved by the Advisory Board and consistent with the requirements of state licensure.
  13. Maintains professionalism and establishes rapport with students, parents, and school faculty.
  14. Establishes effective working relationships with Health Center staff.

     

     

     


    Qualifications

    1. Master’s degree in social work licensed to practice in the State of Illinois.
    2. Previous experience in community health is preferred.
    3. Successful experience as a social worker in the field of child adolescent health.


    Essential Functions/Physical Requirements:
    1. Ability to effectively communicate, verbally and in writing.
    2. Tactile, visual, auditory and olfactory senses intact.
    3. Manual dexterity and strength sufficient to perform medical procedures.
    4. Ability to drive a car.
    5. Ability to deal effectively with high levels of stress.
